Save time and fill out our registration paperwork in advance and schedule your appointment from the comfort of your own home. It’s easy to do and can be completed entirely online. If you are under 18 years old, you must have a parent or guardian submit any personal information on your behalf.
This is a two step procedure, please complete all the Form (Step 1) and watch the Videos (Step 2) to save the maximum amount of time when you arrive for your appointment.
Step 1 -Fill Out Registration Form
The information you submit using the online form will be encrypted and sent to Dr. Greco’s office securely.
Alternatively, you can print the registration form out, and bring it with you to your appointment.
Step 2 – Watch Videos
To help educate our patients on their upcoming procedure, please watch the corresponding video for the procedure you will be having done. You may skip this step if you have already watched the informational video for your procedure.
